Documentation required for new patients

  • Chart notes from a doctor or therapist explaining the need for equipment being ordered
  • Detailed Written Order which must Include
    • Beneficiary's Name
    • Physician's Printed Name
    • Date of the order and the start date (if the start date is different from the date of the order)
    • A detailed description of the item(s)
    • Physician NPI
    • Physician signature and date
    • Duration of need
  • Face to Face note signed by the physician stating diagnosis which requires the item(s) being ordered.
    • History and physical exam notes
  • Medical records include documentation of a face-to-face encounter between the beneficiary and the ordering practitioner that occurred within 6 months prior to completion of the Detailed Written Order & Sleep Study
  • A sleep study performed after the initial office visit examination and prior to delivery. The sleep study must be interpreted by a physician who holds either:
    • ABSM; or, ABMS; or, completed residency or fellowship training by an ABMS; or,
    • Active staff membership of a sleep center or laboratory accredited by AASM, ACHC or TJC, formerly the Joint Commission JCAHO.
